As a minimum, please provide the following information when requesting a new report:
1. Report Name
2. Selection Criteria (Select by Terminal, Customer, Driver, Truck, etc...)
3. Does you report need to select date using a date or date range?
a. Does it need a starting and ending date range or just a single date?
b. When data element are we using for the date? (end date, delivery date, settled date, etc...)
4. When data elements do you want to include in the report? (Trip, Driver, Truck, Vin, etc...)
a. Provide a spreadsheet or some other document with a sample of what you would like the report to look like.
5. Would you like the report optimized for a PDF, Excel or some other output type?
